Character Profile Form submission: Create a Character Profile

Hi!

Massive apologies for the huge delay in this review!

Just a couple of things that requiring a quick fix here:

- the link to the plaque appears to be broken
- the links to the mares in the first three slots appear to be broken
- please edit the stat tracker permissions so we can view it
