Form submission: The Stat System Confirmation Form

Hey there! I have a few corrections before I can get this cutie confirmed!

• The Intelligence Potion application link is currently click which does not pertain to your EB. Please update the link to the correct application!
  
• Swan Lake would be worth 2 stats (+1 Headshot, +1 EC) please update that!
  
• Please remove [MQ] Into the Mouth of Hell * from the stat tracker. Since you are not counting stats for it thus making it worth 0 points, it should not be in your stat tracker as it causes confusion!
